The European partnership Transforming Health and Care Systems (THCS) has now opened its third transnational call “Better care closer to home: Enhancing primary and community care”. Forte will support this call with approximately 15 million SEK, during the years 2025–2027. In this call applicants have to submit a pre-proposal to THCS no later than 30 January 2025.

THCS has been established as a European Joint Program partnership co-funded from the European Commission. The aim of THCS is to coordinate and optimize research and innovation efforts in Europe and its partners countries supporting health and care systems transformation. As a Swedish partner, Forte has a special responsibility to coordinate and help develop the partnership program.

Objectives of the call

The aim of this call is to fund research and innovation projects that strengthen primary and community health and care systems and provide policy and decision makers with the necessary knowledge and tools to govern the transitions needed in the primary and community care sector. Projects funded in this call should contribute with financial, organisational, and practice-based service innovations that promote the transformation of health and care systems and contribute to faster exchange of best practices across different countries and regions.

About the grant

In this call it is possible to apply for projects with a minimum of one year, and a maximum of three years duration. Each consortium must include a minimum of three eligible partners.
