Healthcare of the Future 2025 (THCS)
The European partnership Transforming Health and Care Systems (THCS) has opened its third transnational call “Better care closer to home: Enhancing primary and community care”. Forte will support this call with approximately 15 million SEK, during the years 2025–2027.
Granted applications
The THCS Call Steering Committee (CSC) has decided to award funding to 29 proposals in this call. Nine of these involve Swedish participation and are funded by Forte.
Show listTHCS has been established as a European Joint Program partnership co-funded from the European Commission. The aim of THCS is to coordinate and optimize research and innovation efforts in Europe and its partners countries supporting health and care systems transformation. As a Swedish partner, Forte has a special responsibility to coordinate and help develop the partnership program.
Objectives of the call
The aim of this call is to fund research and innovation projects that strengthen primary and community health and care systems and provide policy and decision makers with the necessary knowledge and tools to govern the transitions needed in the primary and community care sector. Projects funded in this call should contribute with financial, organisational, and practice-based service innovations that promote the transformation of health and care systems and contribute to faster exchange of best practices across different countries and regions.
About the grant
In this call it is possible to apply for projects with a minimum of one year, and a maximum of three years duration. Each consortium must include a minimum of three eligible partners.

Timetable for the application
- The call opens: 26 November 2024
- Deadline for submission of pre-proposal to THCS: 30 Januari 2025
- Deadline for submission of full-proposal to THCS: 19 June 2025
- The call opens for registration in Prisma: 12 June 2025, 10:00
- Deadline for registration of full proposal in Prisma: 26 June 2025, 14:00
This call is administered by THCS. Dates may be subject to change as planning and work progress within the partnership programme.
Application
Proposals should be submitted through THCS online submission platform. Further details of the research topics and how to apply can be found in the call text on the THCS website.
Applicants whose pre-proposal has been granted by THCS must submit a full proposal to THCS no later than June 19.
Swedish applicants must also register the Swedish part of their THCS-project to Forte in Prisma no later than June 26.
